Event Outline:
Delegates will be required to attend both parts of this event.
Part 1: The session will explore the graduated response, including how and when to implement and record the various elements of the graduated response. This includes environmental and practice changes, SMART targets and referrals to other agencies and more.
Part 2: This session will focus on writing effective SMART targets, utilising cycles of Assess, Plan, Do and Review (APDR). How to deliver targeted plans effectively and who is the best person to do this and barriers and obstacles to implementation and how to address these.
Learning Outcomes:
Part One:
- To understand the graduated response, and a practitioner’s role within this.
- To understand how the graduated response relates to an Education, Health and Care Needs Assessment.
- To understand the Assess and Plan elements of APDR.
- To know when to seek external advice and support and how to use the local offer.
Part Two:
- To be able to write effective SMART targets and use these as part of APDR
- To understand the Plan and Do elements of APDR
- To develop a toolkit of strategies for effective delivery of the graduated response, APDR and common barriers and obstacles
